Abstract
Background: Thyroid cancer is a rare disease in pediatric patients (0.7% of tumors); and the most common endocrine malignancy in pediatrics. Only 1.8% are observed in patients aged < 20 years, with a higher prevalence recorded in women and adolescents. Unlike adult patients, children with thyroid nodules typically present as neck masses with no associated symptoms. The following case details an aggressive presentation of pediatric papillary thyroid cancer and need for updated guidelines for the management of papillary thyroid carcinoma (PTC) in paediatrics.  Case Report : A 5 - years - old boy came with complaints of a lump in the neck since the age of 3 years which was getting bigger accompanied by enlarged lymph nodes in a both side. There was no family history of thyroid cancer and no history of irradiation. Total thyroidectomy and selective neck dissection were performed, and ablation therapy will be planned and hormonal therapy. Conclusion: Due to the difference in pathophysiology between thyroid tumors in children and adults, a unique approach to PTC management is to be implemented. Data on diagnosis, surgery, radionuclide, and endocrine therapy are discussed, and the further trials are required for a better understanding of risk factors and the management plan for therapy.
